High marks for iBOND® Self Etch

University of Mainz tests self-etching all-in-one adhesive

In a two-year in vivo study the Department for Operative Dentistry at the Johannes Gutenberg University in Mainz, Germany, has tested the marginal properties of iBOND® Self Etch by Heraeus. The self-etching all-in-one adhesive obtained consistently good assessments in the final re-evaluation in terms of marginal adaptation, marginal discoloration and postoperative sensitivity.
Contemporary self-etching adhesives facilitate work in many fields of clinical practice since they combine significantly easier application with excellent adhesive properties. Due to integration of conditioning in the primary adhesive they provide etching, priming, bonding and desensitizing in one single step. Without compromises regarding adhesion to enamel and dentin they represent an easy-to-use and reliable alternative to one bottle etch&rinse adhesives with phosphoric acid etching.

Clinical evaluation of class III fillings


The aim of the clinical study at the Department of Operative Dentistry of the Johannes Gutenberg University in Mainz was to establish whether iBOND Self Etch self-etching adhesive could guarantee clinical and aesthetic marginal integrity of class III restorations over a two-year study period. Therefore 72 class III fillings were placed in 38 patients until March 2007. The cavities were preconditioned with iBOND Self Etch and filled with Venus microhybrid composite by Hereaus. After 24 months 70 fillings were available for final investigation. The clinical acceptability of the fillings was assessed according to the Ryge/CDA-criteria. Those decide if a filling is clinically acceptable or not. Thereby very good results are marked with “Alfa” for most criteria. “Bravo” results differ from the ideal filling conformation for a certain degree, but are still clinically acceptable. In the group of non-acceptable fillings there are further differentiations between a preventative exchange indication of the filling (“Charlie”) and the immediate intervention indication (“Delta”).

Two-year results attest good marginal qualities


After 24 months all 70 re-evaluated restorations showed clinically and aesthetically good results for all criteria which are prerequisites for an excellent adhesive system. The assessment criterion of marginal adaptation was always within the anticipated parameters of an effective adhesive system with 89 per cent “Alfa” (A = optimal) and 11 per cent “Bravo” (B = acceptable) assessments. Regarding marginal discoloration 84 per cent of the fillings obtained A and 16 per cent B marks. 96 per cent of A and 4 per cent of B ratings for postoperative discomfort also indicate excellent sealing of the dentin surface and a reduction in technique sensitivity. Concerning the Ryge-criteria iBOND Self Etch showed a success rate of 98.5 per cent after the two-year follow-up examinations. It thereby exceeded the minimum requirement for composite restorations of 95 per cent after two years.
On the whole, principal investigator of this trial Professor Claus-Peter Ernst concluded that iBOND Self Etch ensures good clinical and acceptable aesthetic marginal results for class III restorations after 24 months.
